FOR VALUABLE CONSIDERATION, receipt of which is hereby acknowledged, I hereby grant the undersigned photographer ("Photographer") the irrevocable right and permission, throughout the world, in connection with the photographs he has taken on me, or in which I may be included with others, the following:

(a) the right to use and reuse, in any manner at all, said photographs, in whole or in part, modified or altered, either by themselves or in conjunction with other photographs, in any medium or form of distribution, and for purposes whatsoever, including, without limitation, all promotional and advertising uses, and other trade purposes, as well as using my name in connection therewith, if he so desires; and

(b) the right to copyright said photographs in his own name or in any other name that he may select. I waive the right to inspect or approve any use thereof.

I hereby forever release and discharge Photographer from any and all claims, actions and demands arising out of or in connection with the use of said photographs, including, without limitation, any and all claims for invasion of privacy and libel. This release shall inure to the benefit of the assigns, licensees and legal representatives of Photographer, as well as the party(ies) from whom he took said photographs.

I represent that I am over the age of eighteen years and that I have read the foregoing and fully and completely understand the contents hereof.

_________________________, successors, affiliates and assigns (the "Photographer') provides adult entertainment and our work is intended for consenting adults worldwide. You understand and agree that the modeling work you are about to engage or have engaged in, and material with which it will be distributed, contains adult and sexually oriented material.
RELEASE AND WAIVER
For and in consideration for the payment I receive for my engagement as a model by the Photographer in the event such Photographs are used or sold, and in further consideration of the publicity to be received by the undersigned by the use of such Photographs, I hereby give the Photographer, his or her or its agents, representatives and assigns, those for whom he or she or it is acting, and those acting with his or her or its permission or authority, and his or her or its respective agents and employees, and any person or entity who may hereafter publish the films or photographs or digital images (hereinafter collectively referred to as the "Users") the absolute right and permission to copyright in its own name, use, reuse, publish, republish, exhibit, display, print and reprint in advertising material, television and motion pictures, magazines, books, video recordings, audio recordings, Internet, CD Roms or other media, known or unknown at this time, and for any other purpose as they may see fit, in their sole and absolute discretion, the films, photographs, video recordings and audio recordings, negatives, prints of every kind and nature, illustration, portraits made from the photographs, pictures, designs, paintings and drawings of every kind and nature heretofore taken, or taken this day, or hereafter taken, including, without limitation, reproductions thereof in which I may be included in whole or in part (hereinafter collectively referred to as the "Photographs"). Such Photographs may be used in any way by the Users for purposes of trade, advertising, publicity or promotion of any kind without restriction. The failure of Photographer to pay for such use shall not be deemed as a failure of consideration by any other Users. World rights and multiple usages are hereby authorized.
I hereby give the Photographer, and those acting under its permission or upon its authority, permission to copy, publish, perform, publicly display, distribute, sublicense, modify, sell, create derivative works from and/or use images of me, and all negative, transparencies, prints, or digital information pertaining to them, in any format, with my legal or professional name and or/endorsements, together or separately, in any web site, media or publication owned, produced or controlled by the company and to make any changes or additions to such images in any manner as the Photographer shall desire in their sole discretion. I also understand that editorial matter, advertising or other content may accompany these photos.
I hereby waive any right to inspect or approve the Photographs or the editorial or advertising copy or printed matter that may be used by the Users in conjunction therewith. I further waive any claims that I may have to the eventual use to which it may be applied. Such Photographs may be used, in the sole discretion of the Users, with or without my name, or a fictitious name and with fictitious or accurate quotations, interviews or biographical material. The Users may use such the Photographs and accurate or fictitious statements attributed to me which may constitute endorsement of services or products, in the sole discretion of the Users.
I hereby release, discharge and agree to defend, indemnify and save harmless the Photographer and Users, their legal representatives, owners and creators, contractors, shareholders, directors, officers and employees, agents, licensees, successors and assigns, and all parties acting under their permission, or with authority from them or those for whom they are acting, from and against any and all losses, damages, costs, charges, attorneys' fees, recoveries, actions, judgments, penalties, expenses and any other loss whatsoever which may be obtained against, imposed upon or suffered by all or any of them which may arise from this engagement and the use of such Photographs, even should the same subject be to ridicule, scandal, reproach, scorn or indignity, and from any liability as a result of any distortion, blurring or alteration, optical illusions or use in composite from, either intentionally or otherwise that may occur or be reproduced in the taking, processing or reproduction of the finished product, or its publication or distribution or which may arise from any breach of any warranty, representation, covenant or agreement made by me. I waive any claim that I may have for alleged violation of privacy, defamation or libel by the use of such Photographs. I agree not to bring any claim, now or in the future, of any kind against the Photographer, or any host, distributor, retailer, vendor, or any contractors, agents, shareholders, directors, officers and employees thereof. I hereby waive and agree not to bring any and all claims I may have now or hereafter, of which I am aware and of which I may not be aware, including but not limited to rights of privacy, publicity, use of my name, copyright, trademark, service mark, trade name, civil and or other claims.
I voluntarily and knowingly have consented to engage in physical nudity or sexually explicit acts for recording, publication, sale and distribution, electronically or in paper form, and assume all risks of doing so. I understand that I may be subject to publicity, embarrassment, shame, ridicule, attention and more as a result. I am solely responsible and accept full responsibility for my actions. I do not find images of nude adults, adults engaging in sexual acts or other sexual material to be offensive or objectionable, and I will not be offended by engaging in explicit sexual display or viewing such conduct by others during filming and shooting this engagement.
I hereby represent that I am over the age of 18 years and have read the authorization and release prior to its execution. I understand that if I misrepresent my age, or if I violate any of the above agreement, I may be in violation of local, state and federal laws. I have not been induced to sign the same other than by the recited considerations, by any representation or statement made by Photographer, his or her or its agents employees, or anyone acting on his or her or its behalf.
The Photographer, as primary producer, hereby certifies to all Users that the visual depictions covered by this release satisfy the standards under paragraphs (a)(1) through (a)(3) of Section 75.7 of the Regulations implementing 18 U.S.C. Subsection 2257 and do not depict actual sexually explicit conduct.
